Chelsea have confirmed that the club has agreed a fee with AC Milan for Andriy Shevchenko.
A statement on the club's official website read: "Chelsea Football Club has agreed terms for Andriy Shevchenko to return to AC Milan. Details of the agreement will remain confidential.
"The move is subject to a successful medical. Chelsea Football Club would like to thank Andriy for his contribution during his two seasons in England."
The Blues signed Shevchenko in the summer of 2006 for an undisclosed fee in a deal that broke the club's transfer record. In two seasons with the Stamford Bridge outfit, the striker recorded 22 goals in 76 appearances.
The move sees the Ukrine interntional return to the club he first joined in 1999. He is their all-time second highest goal scorer with 173 goals in 296 matches.
